New patients

We welcome new patients and would be happy to give you a Practice booklet to help you decide if we are able to provide the sort of service you are looking for. If you have your medical card, please hand it to the receptionist.

If you have lost your old card, complete a registration form, available at reception. Patients are registered with one of the seven partners in the practice but you may see whichever one of us you prefer. You will be asked to complete a short medical questionnaire, one for each of your household, which will help us look after you until your GP notes arrive from your previous practice.

If you have a medical condition such as diabetes, asthma, heart disease or something which needs regular monitoring, please make sure this is indicated on the questionnaire. Please also let us see your most recent repeat prescription slip from your previous practice.
